I think Stevie and her ex, abusing husband is not just a coincidence. Trevor was her husband, but he wasn't a good person even before the accident.
However Stevie also wasn't a good partner, and I think so because she mentioned she had never made a breakfast for her ex.

So after the accident they broke up and Stevie became a prostitute. One year later they met regularly and Stevie again fell in love with him and wanted to fix their lives. When she felt like they were again together, she wanted to make everything better, so made a breakfast for Trevor.

When she got a call from her "psycho ex", I think she got a call from a client and it refers to that she became prostitute because of him. That's why she said it this way because this "ex" is him, and I think she looked at him confused and upset like she didn't want to be in this situation.

In the same scene he said "I haven't slept in a year", then Stevie chuckled like it's because of her, because they had been apart.

I felt like she knew the entire time something is not okay with Trevor, but she loved him that's why he was more important than her clients.

Trevor didn't remember but felt himself safe with her.

In my opinion the biggest hint was Ivan's boots in her apartment before he found the picture in the frame at the end of the movie. She explained the picture, but those boots were clearly refers to his old personality.

Another quick scene that I don't see the connection is when he is chasing Ivan through the tunnels in the mountains and memorizes his plate number.
My thoughts: The first trip to the police station was only in Trevor's mind. When he later turned himself in, the same police officer and detective were present. The subsequent chase through the underground was Jean Valjean running from Javert.

@Bob Wiser,
I agree with you on Ivan representing the devil, and the fingers of Ivan's toes could represent the boy's (Nicholas) toes at the accident? Also, I was thinking about things that happened when Trevor chose his world of not sleeping, he slipped into a subconscious state that karma was acting out its way of having what he did in the accident, things happened to him in a slightly different way. But I'm a bit confused about all the fish heads in the top freezer? I know it was from his fishing trip, but still? *Kuddos to you Barry for the analysis! =)

Hi Barry, one thing we picked up on in the interview room with the insurance guy at his job after the accident at his job…the photograph shows managed fingers His co-worker lost his arm , not fingers. Do you think that has any bearing on Ivan’s hand?

Awesome movie. Such a commitment from Christian Bale deserves an ovation and an award.
I really liked how the plot developed.
And just a thought:
His guilt of making a hit and run disappears. But he did it, he killed the boy. How will he face that?
