Sylvia was the nurse in the movie. The comatose girl was Ana.
Ana meets Tom in the field as other souls are walking into the afterlife near the. Tom tells Ana that she needs to live, then turns and walks into the afterlife, leaving Ana behind. The scene switches back to the ICU where Ana and Richard have both flatlined. Ana’s heart monitor has spontaneously started up again and her eyes open as Nurse Sylvia and the doctor excitedly watch her awaken from her coma.
So Ana does not die in the end.
Richard dies.
