It’s all about Sarah and if she chooses to keep the pills down. Forget the simulation. Forget the Guide. At its core it’s all about her choice. We see her cough up the pills and then take them down to her favorite place at the end and then cut to black. What choice does she make? I think the ending was hopeful. Perhaps she tosses those pills away and chooses to be a part of life again.

Or I’m totally wrong and it is a simulation. Who knows?? I’m ok with not knowing.
